What we do
The Partnership involves all health and social care organisations work closer together than ever before across the borough to improve services for you.
Nurses, social workers and the voluntary sector, will look at new ways of supporting and treating patients at home or in the community, helping them to stay well and be happy.
The Borough has five Neighbourhood areas, with teams located in each one to deliver the support which is needed in that particular area. Local GPs support the work of the teams.
